Network 15 and Network 18

Welcome to ESRD Networks 15 and 18’s ESRD Online system. Once logged in, your facility will have access to resources, webinars and self-reporting activities designed to support CMS goals and quality improvement efforts.

Monthly assignments are due by the 10th of the month for the previous month.

Important: Log in through the ESRD Online Portal. You cannot log in from this page.
To access ESRD Online:

1. Go to:  https://portal.esrdnw.org/.   
2. Log in to the ESRD Online Portal using your username and password.
3. Select Learning Center.
4. Open Quality Improvement Activities Hub 2026-27.
